Job summary

Northern Lakes CMH Authority in Traverse City is seeking an Intensive Care Coordination with Wraparound (ICCW) specialist to coordinate services for children with severe emotional disturbance or developmental needs. The role involves assessment, planning, and collaboration across health, education, and social systems.

The position requires Wraparound facilitation, crisis and transition planning, and documentation in a community-based setting, with statewide ICCW collaboration and fidelity

Qualifications

  • Bachelor's Degree in Human Services from an accredited university, or a Degree in Nursing.
  • Licensed with the State of Michigan as: Preferred LBSW/LLBSW; Considered RN; Non-licensed.
  • Ability to be credentialed by NLCMHA to provide services in this professional discipline.
  • One year experience in the provision of direct services to children and families.

Responsibilities

  • Provide direct clinical services to registered consumers, primarily children and their caregivers.
  • Engage in Wraparound planning, team facilitation, and service coordination.
  • Complete assessments, IPOS with families, and develop individualized intervention plans.

Description

INTENSIVE CARE COORDINATION WITH WRAPAROUND | TRAVERSE CITY

Professional position to facilitate the Intensive Care Coordination w/ Wraparound (ICCW) service for children with severe emotional disturbance (SED) or intellectual / developmental disability. These child consumers may also be involved with or have needs being addressed by other public agencies, juvenile justice, child welfare, and education. The facilitator will work collaboratively with these systems, as well as with both the child and adult systems within NLCMHA. Responsible for direct assessment and service provision with families, maintaining program statistics and developing required reports, communication and collaboration with the community and various funding sources. This position will be involved in collaboration and consultation with other community human service agencies and schools. Services may be provided in the office, the family home, or in other community-based locations. Will work closely with statewide ICCW system through MDHHS and participate in Wraparound fidelity monitoring activities, coaching, outcome measurement, and quality improvement initiatives as required by MDHHS.

ESSENTIAL FUNCTIONS:

  • Provide direct clinical services to registered consumers, primarily mentally ill or developmentally disabled children, adolescents, and their caregivers. Direct services are expected to approximate 30% of the normal workweek.
  • Demonstrate knowledge of developmental disabilities, person-centered practices, Home and Community Based Service principles, and coordination across developmental disability, behavioral health, educational, and community support systems.
  • ICCW care coordination services will include assessment, engagement, assessment, Wraparound planning, team facilitation, service coordination, monitoring, advocacy, and transition planning.
  • Will provide individual and family supportive contacts, crisis intervention, coordination of care, and facilitation of services to assigned clients and their families.
  • Facilitate Child and Family Team meetings utilizing Wraparound principles including family voice and choice, youth-guided planning, inclusion of natural supports, and team decision-making.
  • Facilitate and document all phases of Wraparound process, including engagement, planning, implementation, monitoring, and transition.
  • Identify, engage, and strengthen natural supports and community resources to build long-term sustainability for the family.
  • Develop and facilitate transition plans that promote long-term stability and reduce reliance on formal services.
  • Participate on the Wraparound Community Team.
  • Participate in MDHHS required Wraparound training. Must complete additional SED Waiver requirement training.
  • Complete training and professional development requirements for 24 hour child and adolescent specific training.
  • Conduct comprehensive, quality assessments of both children and families as well as completing the IPOS with the family.
  • Assess each client/family referred, and will develop comprehensive, quality individualized/family community intervention plans.
  • Ensure family assessments and reports are completed in a timely manner and reflect delivery of quality intervention.
  • Ensure funding reports and other required documentation are completed and submitted according to guidelines in a timely fashion.
  • Interface with the business office in monitoring and processing billings for payment of services.
  • Attend team meetings, supervision meetings, and other meetings as required.
  • Work cooperatively with all community agencies and other providers to promote provision of quality, community-based services.
  • Maintain agency paperwork and clinical record documentation, i.e., CAFAS/PECFAS, and/or MichiCANS, annual assessments, person centered plans, status reports, progress notes, and vouchers, in a timely and accurate fashion.
  • Participate in quality assurance programs and in periodic evaluation of performance and outcomes of service delivery.
  • Comply with mandatory reporting requirements in situations of suspected child abuse or neglect.
  • May participate in interventions, assessments, and other aspects of crisis services during and after hours as necessary.

QUALIFICATIONS:

  • Bachelor's Degree in Human Services from an accredited university, or a Degree in Nursing

PAY:

  • Licensed - $58,399 - $66,899
  • Unlicensed - $53,270 - $60,270
