NORTHERN LAKES COMMUNITY MENTAL HEALTH is seeking an ACT Case Manager Specialist in Grayling to join the Assertive Community Treatment/IDDT team. The role ensures continuity of services and carries out the six case management functions in community settings to support individuals with serious mental illness and co-occurring conditions, helping them achieve recovery goals, independence and community tenure.
Reporting to the Operations Manager, the position requires Michigan licensure
ACT CASE MANAGER SPECIALIST - GRAYLING As a member of the Assertive Community Treatment (ACT)/Integrated Dual Disorder Treatment (IDDT) team, the Case Manager Specialist is responsible for ensuring continuity of services, with a primary responsibility for assuring that the six case management functions are carried out: assessment, treatment, planning, linking, monitoring, and advocacy with potentially more challenging consumers. Services are delivered in community settings and are designed to assist individuals with serious mental illness and co-occurring conditions in achieving their recovery goals, increasing independence, improving functioning, and maintaining community tenure. The Case Manager/SUD Specialist is supervised by the Operations Manager as assigned. This position has no direct supervisory responsibilities.
