Job summary

Hampton-Newport News Community Services Board is seeking a Child & Adolescent Intensive Care Coordination Case Manager in Hampton, VA. The role focuses on clinical assessments, care planning, referrals, and advocacy within schools and community supports.

Annual salary is $53,569. Work schedule is Mon - Fri, with some on-call responsibilities. The ICC Case Manager collaborates with a multidisciplinary team to ensure timely, appropriate services for youth with SED or IDD.

Qualifications

• Bachelor’s degree in a Human Services field such as Social Work, Psychology, or Counseling.
• Two (2) years of experience providing mental health case management or care coordination services for children with knowledge of DSM-5 diagnosis and care management principles.

Responsibilities

• Completing clinical and service assessments.
• Developing individualized service plans.
• Coordinating referrals and authorizations.
• Tracking progress toward goals.
• Maintaining accurate documentation.
• Providing advocacy and crisis response in partnership with the multidisciplinary team.
