Job summary

Mental Health Cooperative in Manchester, TN, is hiring an Intensive Care Manager I to provide continuum of care management for children and youth with serious emotional disturbance or severe mental illness. You will mentor new care managers and ensure coordinated care across hospital discharges and community services.

Responsibilities include developing individualized service plans, crisis planning, and collaboration with families, schools, and other agencies, while meeting productivity

Qualifications

• Bachelor’s degree in a health-related field (counseling, psychology, social work, or related).
• Valid Tennessee Driver’s License.
• Acceptable MVR and criminal background check.
• Personal automobile insurance.
• Cell phone.
• Transportation that seats 4 people.

Responsibilities

• Provide continuous treatment team care management to children and youth with serious emotional disturbance or severe mental illness.
• Mentor new care managers on basic skills.
• Assess consumer needs and develop individualized service and crisis plans with measurable goals.
• Referral/linkage to community resources and advocacy as needed.
• Coordinate care at discharge and during transitions within the agency and community.
