Job summary

West Coast Children's Clinic is seeking an Outpatient and C-Change Intensive Care Coordinator (ICC) in Oakland, CA. This full-time, hybrid position involves coordinating intensive behavioral health services for youth with complex needs, assembling the Child & Family Team, and ensuring thorough documentation.

You will manage up to 19 clients, provide crisis support, and work across community settings to advance individualized care plans in collaboration with families and partners.

Qualifications

• Assessment, development and oversight of intensive behavioral health care plans.
• Facilitate regular Child & Family Team (CFT) meetings.
• Work within a multidisciplinary treatment team to support youth and families.

Responsibilities

• Assessment and Care Planning - Conduct comprehensive biopsychosocial, behavioral health, risk, and functional assessments for clients with complex and high-acuity needs.
• Facilitate Child & Family Team (CFT) Meetings - Regularly schedule, organize and facilitate CFT Meetings with youth/families and their community stakeholders.
• Team-Based Care - Work collaboratively as part of a multidisciplinary treatment team.
• Clinical Care - Manage a caseload of up to 19 clients with behavioral health services for youth and families; document, link youth to resources, support skill-building and crisis intervention.
• Crisis Intervention & Decision Making - Assess and respond to behavioral health crises and emergencies; make appropriate interventions and referrals.
• Cultural Humility - Awareness of impacts of power, privilege, race and SOGIE on self and clients.
• Efficiency and Time Management - Be well-organized, manage time and complete timely documentation.
• Work in Community Environments - Provide services in homes, schools, parks, shelters, and other settings.
