Job summary

Elk Ridge Community Health is seeking a Behavioral Health Consultant to join our clinical team in Arizona. The role focuses on assessment and intervention for health behaviors affecting chronic medical conditions, delivering brief, solution-focused interventions, and coordinating with primary care.

The BHC will support care continuity, staff education, and crisis response within a collaborative, multidisciplinary setting.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology or related field.
• Three (3) years’ post graduate experience working with behavioral health patients in a clinical setting.
• Current Arizona Behavioral Health License at the independent level (LPC, LCSW, Clinical Psychologist).
• Current Basic Life Support (BLS) certification for health care providers.
• Level I fingerprint clearance card (valid or applied for within seven days of start).

Responsibilities

• Provides focused quality clinical services to patient health and behavioral concerns, including assessment and intervention.
• Supports referral processes and warm hand-off requests.
• Cultivates education and self-management skills for patients with comorbid medical and behavioral health conditions.
• Collaborates with primary care providers and multidisciplinary teams to address health conditions.
• Maintains accurate electronic health records in compliance with HIPAA and Elk Ridge policies.
• Provides education to teams on behavioral health assessment, safety and risk, and evidence-based practices.
