Job summary

Mountain Park Health Center in Phoenix seeks an Integrated Services Navigator to connect patients with behavioral health or nutritional counseling, schedule appointments, and navigate barriers to care with culturally competent service.

You will conduct face-to-face screenings, document in EMR, and collaborate with providers to ensure timely access while maintaining dependable attendance and patient satisfaction.

Qualifications

• High School education or GED required.
• 1+ year experience in Electronic Medical Records.
• Valid AZ driver’s license.
• Bilingual in English and Spanish.

Responsibilities

• Conducts face-to-face screenings to schedule patients eligible for behavioral health or nutritional services.
• Identifies, evaluates, and resolves barriers to care, scheduling and completion of integrated services appointments.
• Utilizes motivational interviewing tools to encourage patients to schedule and complete integrated services visits.
• Maintains accurate and complete documentation in patient records.
• Evaluates patient concerns, complaints, and trends to recommend changes to accommodate patient needs and eliminate barriers.
• Maintain regular and predictable attendance.
• Performs other duties as required.
