Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts to monitor, follow up, and manage care for dually-eligible enrollees with complex needs. You will coordinate health services and community resources to improve outcomes and quality of life.

The role involves leading an interdisciplinary team, overseeing long-term service utilization, and ensuring effective communication among providers, families, and enrollees. Travel across the territory is required.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs.
• Case management experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build care management relationships.
• Coordinate with healthcare providers and community resources for seamless care transitions.
• Lead interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
