Qualifications

• Current Registered Nurse license in Ohio.
• BSN required; Master’s degree preferred.
• Experience in case management, care coordination, or discharge planning is desirable.

Responsibilities

• Identify and assess patients and families with social, psychological and environmental needs.
• Develop intervention plans addressing priorities and coordinating supportive services.
• Assist patients and families to improve social functioning and transition planning.
• Collaborate with physicians, nursing, and ancillary services for safe, next-phase care planning.
