Responsibilities

• Provide integrated, community-based care coordination for adults, children and families.
• Coordinate care across physical health, behavioral health and social needs; engage patients and families as partners.
• Maintain clinical records and ensure compliance with agency policies and standards.
• Support transportation, housing, and service access to improve treatment outcomes.
• Collaborate with multidisciplinary teams and community partners to meet treatment goals.
