Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager - BH to monitor and manage care for dually eligible enrollees with complex medical, behavioral, and social needs. You will lead the interdisciplinary team, conduct essential assessments, coordinate with providers and community resources, and travel to member homes and sites to ensure seamless care.

The role requires a Master’s in social work or mental health counseling and an active license (LCSW/LISCW/LMHC).

Qualifications

• Master's degree in social work or mental health counseling required.
• Independent license (LCSW/LISCW/LMHC) required.
• Experience in care coordination or dual-eligible populations preferred.
• Medicaid/Medicare knowledge is preferred.

Responsibilities

• Lead the interdisciplinary care team to develop holistic care plans.
• Perform comprehensive and functional assessments in a timely manner.
• Coordinate with healthcare providers and community resources for enrollee care.
• Engage enrollees in care plan development and updates.
• Travel to member homes and community sites for visits.
• Ensure continuity of care and prevent readmissions.
• Educate members about benefits, services, and preventive care.
• Adhere to NCQA and Care Management standards.
