Qualifications

• Master's degree in social work or mental health counseling; independent license required.
• BH Clinician with the ability to independently serve people with complex needs; experience in care coordination preferred.

Responsibilities

• Engage enrollees in homes and community settings to establish complex care management relationships.
• Coordinate with ICT and healthcare providers to create holistic care plans.
• Lead the interdisciplinary care team to address medical and non-medical needs.
• Travel to member, provider, and community visits as required.
• Educate members about benefits, managing chronic conditions, and preventive care.
