Job summary

IU Health is seeking an Integrated Care Management Manager to lead case management and social services, ensuring effective care for patients and successful transitions. The role emphasizes collaboration with nurses, physicians, social workers, and hospital staff to align care plans with community resources.

The ideal candidate will hold an advanced nursing degree or social work credentials, possess strong leadership and clinical assessment skills, and drive quality improvement across the care

Qualifications

• Requires Bachelor's in Nursing with Master's preferred, or Master's in Social Work.
• Requires active RN license in Indiana or NLC RN, or LCSW preferred (within 2 years if SW).
• Case Management certification preferred.
• 5–7 years of relevant leadership experience in care transitions.
• Strong clinical assessment, planning, and goal development skills.
• Knowledge of Case Management principles and community resources.

Responsibilities

• Provides direction and leadership in case management and social services to ensure effective patient care.
• Coordinates services to promote efficient care transitions and appropriate resource use.
• Collaborates with multidisciplinary teams to integrate services and improve patient outcomes.
• Supports program direction, policy development, and quality improvement initiatives.
