Thresholds is seeking a Service Coordinator to support mental health recovery by coordinating care, linking clients to housing, medical care, and community resources. You will build rapport with clients weekly, gather information, and complete assessments in various settings across Chicagoland.

You will work in a collaborative culture with opportunities for professional development, including clinical supervision, to deepen skills in harm reduction and housing first models.

Qualifications

• Bachelor’s degree in social work, psychology or closely related field required.
• Master's degree in social work, counseling or a closely related field preferred.

Responsabilités

• Supports the mental and physical health care of members on an assigned caseload.
• coordinates care with member’s mental health, physical health, and/or substance use providers.
• Collaborates with Thresholds Housing Administration and partners to ensure service delivery and outcomes.
