Job summary

ICL is seeking a Case Manager at our Transitional Veterans Residence in New York to organize comprehensive service plans and coordinate with housing, mental health, and healthcare providers. You will work closely with residents to develop individualized plans and support their progress toward goals.

The role involves direct services, crisis intervention, advocacy, and collaboration with a multidisciplinary team, with a standard 9:00am–5:30pm schedule and occasional late day shifts.

Qualifications

• Education and Experience: BA with practicum in target population or BA + 1 year case management or relevant human services experience.
• AS in health or human services with 3 years case management experience.
• HS diploma/GED with 3 years direct services to mentally disabled/homeless individuals.

Responsibilities

• Organize comprehensive service planning and assessment and ensure plan implementation.
• Provide linkages/advocacy with housing, mental health, substance abuse, and healthcare providers.
• Develop residents' service plans with program staff for holistic care.
• Conduct orientation for new residents to explain clinical services and ICL programs.
• Conduct assessments and document medical, psychological, and social factors.
• Provide individual and group counseling to modify attitudes/behavior and address needs.
• Develop resource information and communicate clearly to residents.
• Assist with discharge planning and crisis intervention as needed.
• Refer residents to community resources and accompany to appointments.
• Facilitate transportation and budgeting training for residents.
