Nottawaseppi Huron Band of the Potawatomi (NHBP) seeks a Behavioral Health Consultant in Battle Creek, MI. You will deliver culturally appropriate behavioral health services within a primary care setting, working on crises, screenings, and integrated care with a multidisciplinary team.

Under supervision, you will support quality improvement, grant reporting, and outreach while maintaining HIPAA guidelines and community partnerships.

Qualifications

• Master’s degree in Social Work or related field and licensed in Michigan.
• Minimum 3 years post graduate practice experience.
• At least 1 year in a team lead or managerial role.
• Familiarity with tribal communities and cultural competency.

Responsabilités

• Provide brief, solution-focused behavioral health interventions in primary care.
• Coordinate referrals with warm hand-offs and follow-up.
• Screen for Depression, Anxiety, chronic pain, PTSD and related concerns.
• Lead or co-lead case conferences with the care team.
• Collaborate with multidisciplinary team to manage complex cases.
• Document in EHR per policy; ensure HIPAA compliance.
• Participate in outreach, grant-related activities and meetings.
• Monitor grant funds and report outcomes with data.
