Job summary

Lake Superior Community Health Center, Inc. is seeking a behavioral health professional to provide integrated behavioral health services within a primary care setting.

The role includes assessments, diagnoses, counseling, and collaboration with the care team to support positive behavioral change. You will coordinate care with primary providers, participate in crisis intervention, and ensure thorough documentation in the EHR while adhering to HIPAA and OSHA regulations.

Qualifications

• Ability to work with an interdisciplinary team and provide services to diverse populations.
• Ability to evaluate, diagnose, and counsel clients independently with sensitivity and professionalism.
• Proficiency in brief, solution-focused behavioral health interventions and documentation in EHR.

Responsibilities

• Coordinate and co-manage individual patient care with rapid feedback to primary care providers.
• Provide on-site crisis triage and intervention as needed.
• Deliver brief behavioral health care and follow-up for positive behavioral change.
• Provide clinical consultation to medical providers on behavioral health and case management.
• Document assessments, diagnosis, treatment, referrals, and follow-up in the EHR according to guidelines.
• Comply with policies, standards of care, HIPAA and OSHA regulations.
• Participate in staff meetings, quality improvement, and collaborate with community organizations.
• Other duties as assigned.
