Job summary

Family Health Services in Idaho is seeking a Behavioral Health Consultant (BHC) to support integrated primary care in outpatient clinics. The BHC delivers brief behavioral interventions for patients with chronic illness to improve disease management, within a collaborative framework and team-based care.

Strong coordination and communication with the care team are essential. Responsibilities include screening, triage, case management, and brief interventions, with opportunities for crisis

Qualifications

• Master’s in Counseling, Social Work, or equivalent; Idaho license in good standing or eligible.
• Experience in interdisciplinary teams and care coordination for chronic illness.
• Excellent verbal and written communication skills.

Responsibilities

• Provide brief clinical interventions and triage in primary care clinics.
• Engage patients, assess, plan treatment, and monitor referrals.
• Collaborate with care teams and community resources.
• Document services in the EHR and participate in performance improvement activities.
