The Nottawaseppi Huron Band of the Potawatomi seeks a Behavioral Health Consultant to deliver culturally appropriate behavioral health services within the Health and Human Services Department. You will work as part of a multidisciplinary team to address psychological symptoms in a primary care setting.

Qualifikationen

• Master’s degree in Social Work or related field such as Professional Counseling or Psychology.
• Must be licensed in Michigan in good standing.
• 3 years post graduate experience in the field.
• 1 year of team lead or managerial experience.
• Experience with Tribal Governments or Native American communities preferred.
• Knowledge of behavioral health medications and primary care integration.

Aufgaben

• Provide brief, solution-focused behavioral health interventions in primary care.
• Compile referrals with warm hand-offs and follow-up for longer-term therapy.
• Screen for Depression, Anxiety, chronic pain, PTSD, aging-related concerns, and more.
• Lead or co-lead case conferences with integrated care team members.
• Collaborate with multidisciplinary team to manage complex care needs.
• Document in the EHR in line with policy.
• Comply with reports of abuse or neglect as required.
• Participate in outreach, education, and grant-related activities.
• Gather data for grant project reports and monitor grant funds.
