Job summary

Northshore Health Centers, Inc is seeking a Behavioral Health Care Coordinator to support the care team in Demotte, IN. The role focuses on integrating behavioral health with primary care, coordinating services, and supporting patients through education and ongoing follow-up.

The coordinator will schedule referrals, assist with transitions of care, and maintain strong communication with PCPs and the care team to optimize patient outcomes.

Qualifications

• Minimum education per Indiana/HRSA guidelines.
• Medical assistant, LPN, APN or RN licensure.
• 1 year of behavioral health and/or coordination experience.
• Valid CPR certification.
• Bilingual preferred in some cases.

Responsibilities

• Schedule behavioral health, substance use, and psychiatric appointments.
• Provide education and resources for mental health and addiction services.
• Coach patients to set SMART goals for self-management.
• Close the loop on referrals to improve access to care.
• Assist with transitions of care from ED or inpatient settings.
• Explain discharge instructions and answer patient questions.
• Coordinate access to individual and family support services.
• Follow up with patients to assess progress and barriers.
