Job summary

The Primary Care Behavioral Health (PCBH) model is integrated into Spira Care's primary care practice in the Kansas City area. The Behavioral Health Consultant (BHC) collaborates with medical providers to address both physical and mental health needs and to promote patient self-management.

The BHC will deliver brief, evidence-based interventions, participate in team meetings, and educate patients about diagnoses and healthy lifestyle changes while ensuring quality documentation and coordination

Qualifications

• Master's degree required in social work or related field.
• Active licensure in KS and MO (LSCSW/LCSW, LMFT/LPC, LCPC) required.
• At least 5 years of behavioral health consulting experience.
• Strong knowledge of primary care integration and EMR usage.

Responsibilities

• Provide brief behavioral and cognitive interventions to treat emotional and physical health concerns.
• Collaborate with care team to develop a common treatment plan and coordinate care.
• Educate patients about their diagnosis and healthy lifestyle changes; promote self-management.
• Respond to crisis situations, assess urgency, and refer as appropriate.
