Job summary

Mountain Family Health Centers is seeking a Behavioral Health Provider to perform Primary Care Behavioral Health (PCBH) duties, including assessment, crisis intervention, and brief therapy. The role requires charting, collaboration with medical and dental teams, and coordination with care staff to support diverse clients.

Ideal candidates hold a Master’s degree in counseling, social work, or related field and Colorado licensure (or eligible). Spanish fluency is highly recommended.

Qualifications

• Master’s degree in counseling, social work, or marriage and family therapy (or higher).
• Colorado licensure required or eligible for Colorado license.
• Minimum of two years’ experience preferred; applicants soon to be licensed may be considered.
• Experience in the medical field prioritized in hiring.
• Spanish fluency highly recommended.

Responsibilities

• Perform PCBH related duties including clinical assessment and problem identification.
• Provide crisis intervention, brief individual and family therapy, and case management.
• Document interactions in medical charting software and coordinate with medical, dental, billing, and care coordination staff.
• Work with a diverse client population across settings and treatment approaches.
