Job summary

Appalachian Mountain Health seeks an Licensed Integrated Behavioral Health Provider to deliver outpatient mental health services within our primary care setting. You will work with MDs, NP s, PAs, nurses, pharmacists, medical assistants, and psychiatry staff in a collaborative team.

You will perform short-term counseling, conduct diagnostic assessments, create care plans, and support clients with resilience-building strategies and coping skills.

Qualifications

• Requires Master’s degrees in social work and mental health counseling.
• Licensed professional credentials in behavioral health are required (LCSW/LCSWA, LCMHC/LCMHCA, PsyD/LPA).
• Experience in a clinical outpatient setting with diverse patient populations.

Responsibilities

• Collaborate within a team of medical and mental health providers to integrate care.
• Conduct diagnostic assessments and develop treatment plans for behavioral health needs.
• Document interactions and outcomes, including referrals, within CMS standards within 72 hours.
• Provide patient education and crisis intervention when needed.
• Engage in brief, evidence-based treatment modalities for a range of conditions.
