Job summary

Qualifications

• Arizona Licensed Behavioral Health Professional.
• Bilingual English/Spanish preferred for patient communication.

Responsibilities

• Provide focused behavioral health services within the primary care clinic.
• Conduct mental health assessments and deliver evidence-based interventions.
• Coordinate care between primary care and behavioral health teams.
• Offer crisis assessment and triage in collaboration with community resources.
• Maintain accurate, HIPAA-compliant patient records and documentation.
• Participate in team meetings, trainings, and quality improvement activities.
