Job summary

Centerstone is seeking a Case Manager to collaborate with the local Health Department and coordinate behavioral health and medical services. Bachelor’s degree in psychology, social work or related field is required; experience with Electronic Health Records is preferred; valid driver’s license required.

Qualifications

• Bachelor’s Degree in psychology, social work or related field required.
• Experience with Electronic Health Record preferred.
• Valid driver’s license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refer, advocate, and link client and family to additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
• Provide supportive and educational case management services to develop daily living and coping skills.
• Develop, implement, evaluate, and update treatment plans with input from the client and family.
• Enter written documentation into EMR in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs.
