Job summary

Centerstone in Florida is seeking a passionate Case Manager to coordinate care for individuals and families across community settings, including offices, hospitals, homes, and schools. You will work with CAT and CORe programs to provide case management, resource linkage, and ongoing support to improve stability and well-being.

Responsibilities include developing and updating treatment plans, conducting assessments, and maintaining timely EMR documentation, while collaborating with local partners

Qualifications

• Bachelor’s degree required with strong organizational and communication skills.
• Ability to develop supportive relationships with persons with mental health and substance use disorders.
• Must have valid FL driver’s license with good driving record and auto insurance.

Responsibilities

• Provide individual and group mental health and substance use disorder case management.
• Refer and link clients to additional services.
• Develop, implement, and evaluate treatment plans.
• Enter documentation into Electronic Medical Records.
• Coordinate with local law enforcement and stakeholders.
• Transport clients using company vehicles.
• Complete required trainings and maintain productivity.
