Job summary

Centerstone is seeking a Case Manager to co-locate with the local Health Department and improve access to behavioral health services for patients in medical care. You will bridge primary and behavioral health needs, education patients, and coordinate resources to support whole-person care.

Qualifications

• Bachelor’s degree in psychology, social work or related field is required.
• Experience with Electronic Health Record preferred.
• Valid driver’s license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refers, advocates, and links client and family for additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
• Provide supportive and educational case management to develop daily living and coping skills.
• Develop, implement, evaluate and update treatment plans with input from client and family.
• Enter written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs.
• Demonstrate knowledge and understanding of ethical standards with patients and community partners.
• Attend weekly staffing and team meetings and complete assessments.
• Assist clients with developing natural support systems.
• Complete all required agency trainings to maintain 100% compliance.
• Maintain departmental productivity expectations.
• Attend and participate in clinical supervision.
