Job summary

The Primavera Foundation is seeking a Resource Specialist to conduct housing assessments in person and by phone, and to complete intake interviews for ES housing programs. You will create and maintain HMIS records with high data accuracy and refer participants to essential services.

Collaboration with staff and partners is essential; cross-training and coordination with ES teams help ensure smooth daily operations and strong client outcomes.

Qualifications

• Bachelor’s degree in related field from an accredited college or university.
• At least one year of professional social services work experience as a resource specialist or case manager.
• Proficiency with Word, Excel, and Outlook.
• Proficiency with computer database systems for entering data and tracking referrals.
• Ability to build rapport with diverse individuals.
• Valid Arizona Driver’s license, clean driving record, proof of insurance, and reliable transportation.
• Current CPR and First Aid certification, or ability to obtain upon hire.
• Level 1 fingerprint clearance card, or ability to obtain upon hire.

Responsibilities

• Conduct housing assessments in person and by phone with accurate referrals.
• Complete intake interviews for ES housing programs and finalize intake paperwork.
• Create and maintain HMIS records and case notes with high data accuracy.
• Refer participants to eligible services and support housing connections.
• Attend required meetings and trainings with internal and external collaborators.
• Cross train with ES staff to ensure coverage of daily operations.
• Maintain knowledge of shelter resources and refer participants to Primavera shelters.
• Maintain professional relationships and confidentiality with participants.
• Adhere to Primavera’s guiding principles: integrity, respect, accountability, compassion, leadership.
• Perform other duties as assigned by leadership.
