Job summary

Kingwood Pines Hospital, part of Universal Health Services, seeks an Intake Nurse / Counselor to perform medical screening and triage for psychiatric and chemical dependency patients. You will coordinate admissions for inpatient, partial, or outpatient programs and facilitate referrals to community resources.

Responsibilities include crisis intervention, nurse-to-nurse signaling, and ensuring accurate documentation and communication.

Qualifications

• Graduate of accredited school of nursing; Texas RN license required.
• Bachelor’s degree is preferred but not required.

Responsibilities

Kingwood Pines Hospital is a 116 bed facility located in the beautiful residential Kingwood community. We are located about 30 miles northeast of the Houston Medical Center. The hospital provides 24-hour emergency services. We serve children, adolescents, and adults for psychiatric and behavioral health. The hospital also has services in intensive outpatient and partial hospital program services.

Position Summary

The Intake Nurse / Counselor provides medical screening and triage to all psychiatric and chemical dependency patients presented for assessment and evaluation; coordinates the admission process for inpatient, partial or outpatient programs. Facilitates referrals for outpatient services and other community resources.

Key Responsibilities

1. Perform initial nursing screening and appropriate clinical assessment on all persons who present for an evaluation.
2. Recognizes signs and symptoms of chemical dependency disorders and applies/utilizes appropriate treatment interventions.
3. Recognizes signs and symptoms of mental health disorders and applies/utilizes appropriate treatment interventions.
4. Assess, direct and intervene in crisis situations.
5. Administer medications as ordered by the physician should the need occur.
6. Perform the nurse-to-nurse screen for all patients transferred in or out of the Admissions Department.
7. Perform and document all pre-certifications on all admissions.
8. Facilitate referrals to outpatient treatment services.
9. Assume personal responsibility for excellent customer service.
10. Respond to telephone and face-to-face inquiries from persons seeking behavioral health services.
11. Document calls and related information on Intake form and enters data into computer system.
12. Stay abreast of organization’s programs and services.
13. Assist Director with performance monitors.

Qualifications

• EDUCATION: Graduate of accredited school of nursing. Bachelor’s preferred.
• LICENSURE/CERTIFICATION: Current RN TX license.
• Current Texas Drivers license
• EXPERIENCE: Minimum of two (2) years experience in psychiatric nursing is preferred.

Shift / Hours

• 9am - 9am - Roating Shifts
