Job summary

Millwood Hospital in Arlington, TX, part of Universal Health Services, is seeking an Intake Assessor/Counselor. This role provides in-person risk assessments to determine appropriate level of care, coordinates referrals and intake, and supports admissions and patient communication.

The Intake Assessor works with the physician, families, and staff, ensuring pre-certifications and proper documentation while maintaining liaison with the UR and Business Office as needed.

Qualifications

Intake Assessor/Counselor

The Intake Assessor is a Licensed Counselor, under the supervision of the Director of Assessment/Referral, provides in-person risk assessment to assist the physician in identification of appropriate level of care for potential patients. The Intake Assessor is the initial contact with referral sources and potential patients, coordinates the referral and intake process, facilitates the admission process and maintains communication with referral sources, families, patients, and unit staff. The Intake Assessor works with the Business Office regarding patient financial data and verification and prior authorization of insurance benefits as needed, as well as with the UR Department for the certification of patients admitted.

Work Hours: Days (7a - 7p)

Job Duties/Responsibilities:

• Inquiry calls are documented and processedin accordance withIntake policies.
• Patients are assessed and stabilized prior to leaving hospital or admission to hospital arranged.
• On calldoctoris notified andassistedwith patientinterview.
• Administrative approval is obtained for all admissions.
• Admissionpaperwork is completedin accordance withhospital policy.
• Intakelog is completed with all information documented.
• Follow-up calls are made to thosefailing to showup for appointments.
• Pre-certification is obtainedin accordance withinsurance requirements.
• Outcome studies forms are completed.
• Guest relations aredemonstratedbyexampleand employees in need of guest relations training are counseled.
• Administration is notified of problems orsignificant events.
• Safety problems areidentifiedand corrective actionstaken.
• Medication is obtained after hours as ordered by the physician with proper documentation of drugs removed and accountabilitymaintained.
• Assures that Core Measures Data (HBIPS) Abstraction Tool is completed as relates to all admissions.
• Other duties as assigned or director by Department Director and/or Supervisor.

Qualifications

Requirements:

Education:

Registered Nurse or Master's Degree in a human relations or other related field.

Experience

Two (2) or more years of experience in Intake assessment activities within an acute psychiatric setting. A minimum of two (2) years' experience in a psychiatric hospital setting including customer contact.

Licensure:

Licensed to practice in the State of Texas as a mental health professional. (RN, LMSW, or LPC)

Additional Requirements:

A working knowledge of the DSM-IV and the Mental Status Exam. A working knowledge of the Addiction and Recovery field. May be required to work flexible schedule and overtime as needed; may be required to work on-call as needed.
