Job summary

Allegheny County's ODS intake team seeks an Intake Specialist to determine eligibility and connect families with resources. You will document case details in the ODS system, map community supports, and coordinate with SCOs, DHS partners, and planning teams. Travel may be required to homes or community sites.

Strong communication, organization, and knowledge of disability eligibility are essential. Bachelor's degree in social work or related field with 1 year of relevant experience is required.

Qualifications

• Bachelor’s degree in social work or related field and 1 year of relevant experience.
• Experience with individuals/families facing intellectual or developmental disabilities preferred.
• Strong communication and organizational abilities essential.

Responsibilities

• Serve as a resource for individuals/families with multiple needs for triage.
• Utilize resources and help locate new resources for clients.
• Enter and maintain information in the ODS Intake system and portal.
• Document resources in client records and follow up with teams.
• Provide intake information and support to Supports Coordination Organizations (SCOs).
• Collaborate with ODS teams and planning to address waiver services as needed.
• Build relationships with community organizations and the PA Family Network.
• Explain rights regarding appeals processes and documentation.
