Job summary

Allegheny County Department of Human Services is seeking an Intake Specialist to determine system eligibility and guide individuals and families to appropriate resources within the ODS framework. The role involves assessment, resource mapping, and helping clients navigate services and waiver options.

The position requires travel within the county, flexible hours, and collaboration with Supports Coordination Organizations and multiple DHS teams to ensure accurate information and effective service

Qualifications

• Bachelor’s degree in social work or related field with 1 year experience in the intellectual disability or related area.
• Ability to travel within the county to meet with individuals and families.
• Experience or ability to enter and manage data in online systems for intake and resource mapping.

Responsibilities

• Serves as a resource for individuals/families with multiple needs for triage.
• Utilizes knowledge of resources and assists in locating new resources.
• Enters information into the ODS Intake and Registration Application and updates the portal.
• Develops a mechanism to document resources in community and service systems.
• Follows up with individual teams to assess resource effectiveness.
• Provides information and support during intake to Supports Coordination Organizations (SCOs).
• Coordinates with ODS teams to plan waivers and services as needed.
• Maintains relationships with community organizations and the PA Family Network.
