Qualifications

• Must have a high school diploma or equivalent.
• Must have at least two years in the health care industry and one year in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have knowledge of Medicare guidelines governing home health or hospice.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines governing processes.
