• Strong understanding of Medicaid/MLTC enrollment processes and payer requirements.
• Excellent follow-up, multi-referral management, and professional communication.

Responsibilities

• Manage and follow up on all referrals through admission and start of care.
• Verify eligibility, authorizations, and payer requirements for home care services.
• Coordinate communication between referral sources, patients, caregivers, plans, and staff.
• Obtain and review all required documentation for admission.
• Ensure compliance with NYS DOH regulations and agency policies.
• Track referral status and maintain accurate documentation in systems.
• Answer calls professionally and provide information on services.
• Collaborate with staffing, nursing, and administrative teams for seamless initiation.
• Maintain relationships with hospitals, MLTCs, physicians, and referral sources.
