Grace Community Care & Homes in New Jersey seeks an Intake and Referral Coordinator to support person-centered services, reliable operations, and accurate documentation within the Central Operations department.

Responsibilities include receiving inquiries, collecting intake data securely, verifying eligibility, and tracking outcomes while collaborating with intake, credentialing, billing and quality teams. This is a part-time, on-site role with a $18/hour wage and PTO/sick leave benefits.

• Experience in healthcare intake or case-management.
• Strong data entry accuracy and confidentiality.
• Effective communication and coordination with multiple teams.

Responsibilities

• Receive inquiries and referrals, explain programs, collect intake information securely, verify eligibility and authorizations, and track referral outcomes.
• Collaborate with leadership, intake, credentialing, billing and quality teams.
• Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow policies and scope.
• Participate in orientation, competency review and quality improvement.
