Job summary

Grace Community Care and Homes provides community-based care and outreach coordination. We are seeking a part-time Community Referral and Outreach Coordinator to build referral networks, present services to hospitals and community agencies, and manage CRM follow-up with a focus on person-centered care and accurate documentation.

Start date is contingent on program approvals and staffing readiness. The final assignment and weekly schedule will be discussed during hiring, with on-site or community

Qualifications

• Healthcare outreach or community-partnership experience.
• Strong phone and writing skills.
• Organized follow-up and respect for patient choice.
• Applicants should provide credentials, experience, availability and a resume.

Responsibilities

• Build and verify referral contacts and introduce services to hospitals.
• Arrange virtual presentations, maintain CRM follow-up and track referral conversion.
• Collaborate with intake, credentialing, billing and quality teams.
• Maintain confidentiality and follow program policies.
