Grace Community Care & Homes is seeking an Intake and Referral Coordinator to support person-centered services and reliable operations. The role will collect intake information, verify eligibility, and track referrals, while collaborating with intake, credentialing, billing and quality teams.

This part-time position in New Jersey offers on-site or community assignments, with pay at $18 per hour. Start date and hours are discussed during hiring, and paid time off is provided under company policy.

Formación

• Healthcare intake or case-management experience preferred.
• Clear communication and accurate data entry required.
• Ability to handle confidential information securely.
