Qualifications

• Minimum of 2 years of experience in home care, healthcare, or a client-focused setting.
• Must reside in Pennsylvania.
• Willingness to travel throughout the assigned coverage area.
• Ability to work occasional evenings and weekends as needed.

Responsibilities

• Conduct initial client intakes and comprehensive home assessments with clients and their families.
• Perform quality assurance and supervisory visits in clients' homes.
• Assess the quality of care being provided and ensure compliance with agency policies and standards.
• Document and report findings from quality assurance and supervisory visits.
• Collaborate with clients, families, and care teams to support positive client outcomes.
