Job summary

Emerest Health of PA in Schuylkill County is seeking an Intake Specialist to support clients from intake through initial care coordination, working closely with families and care teams.

The role is on-site, requires travel within the coverage area, and may include evenings and weekends. Perks include per diem compensation, flexible schedule, mileage reimbursement, and complimentary spa services at our Natrona Heights, PA office.

Qualifications

• Minimum of 2 years of experience in home care, healthcare, or a client-focused setting.
• Must reside in Pennsylvania.
• Willingness to travel throughout the assigned coverage area.
• Ability to work occasional evenings and weekends as needed.

• Conduct initial client intakes and comprehensive home assessments with clients and their families
• Perform quality assurance and supervisory visits in clients' homes
