Job summary

Ray Graham Association seeks a compassionate Service Intake and Outreach Coordinator to be the first contact for individuals seeking services. You will manage referrals, conduct intake screenings, and guide families through admissions and enrollment while maintaining accurate records.

The role collaborates with ISC agencies, state partners, and healthcare providers to ensure a smooth, person-centered experience in Illinois. Office-based with occasional travel within the service area.

Qualifications

• High school diploma or GED required.
• Valid driver's license with an acceptable driving record.
• Minimum of two years of experience in intake, customer service, case management, healthcare, human services, or a related field.
• Excellent verbal and written communication skills.
• Strong organizational, documentation, and time-management skills.
• Ability to handle multiple priorities while maintaining attention to detail.
• Proficiency with Microsoft Office and computerized database systems.
• Ability to work collaboratively with diverse individuals, families, and community partners.

Responsibilities

• Serve as the primary contact for inquiries regarding services and supports.
• Respond to referrals from individuals, families, hospitals, schools, community agencies, and other referral sources.
• Conduct intake screenings and collect necessary documentation.
• Guide individuals and families through the admissions and enrollment process.
• Maintain accurate records of referrals, vacancies, and intake activities in organizational databases.
• Coordinate communication among internal departments to facilitate service placements.
• Assist individuals and families in navigating the Illinois I/DD service system, including available programs and resources.
• Develop and maintain positive relationships with ISC agencies, state agencies, healthcare providers, and community partners.
• Prepare reports and monitor referral and intake data to support planning and service delivery.
• Participate in outreach, advocacy, and community engagement activities to increase awareness of services.
• Ensure interactions are person-centered, respectful, and aligned with mission and values.
