Job summary

Continuum Care of Washington is seeking an Intake Manager to oversee the patient referral and intake process and lead the intake team. The role focuses on building relationships with referral sources and managing insurance verification and authorization workflows.

The ideal candidate has 3+ years in healthcare, preferably home care operations, and strong supervision and communication skills to ensure compliant hospice admissions.

Qualifications

• High School Diploma is required; college degree preferred.
• Proficient in Google Drive/Sheets/Docs.

Responsibilities

• Directs all daily patient referral and intake operations, implementing intake policies.
• Enters referral/patient information into EMR and other applications as needed.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with state, federal, CHAP referral/intake requirements.
• Oversees insurance verification and authorization processes.
• Maintains knowledge of contracts and community resources for patient access.
