Acacia Health in Irvine, CA seeks a compassionate LVN Intake Care Coordinator to manage the intake workflow for home health and hospice services.

You will verify insurance, coordinate with clinical teams, and support Start of Care admissions while ensuring HIPAA-compliant documentation. This full-time, in-office role offers 10:00a–6:30p hours and a pay range of $30–$35 per hour depending on experience.

Qualifications

• CA LVN license required and active.

Responsabilités

• Referral Intake & Coordination: Manage hospice referrals from receipt through clinical handoff.
• Insurance Verification & Authorizations: Verify eligibility and obtain required authorizations.
• Documentation & Data Management: Enter and maintain patient/referral/insurance data; HIPAA compliant.
• Communication & Referral Partner Support: Be primary contact for referral partners and physicians.
• Admission & Scheduling Support: Coordinate admissions and Start of Care with teams; arrange DME & transport.
• Compliance & Quality: Follow HIPAA, support audits and process improvements.
