American Medical Home Health Services is seeking an LVN Intake Coordinator to serve as the clinical liaison for incoming hospice referrals and admissions. You will coordinate referrals, verify eligibility, and ensure timely patient admissions while maintaining HIPAA compliance and Medicare/state regulations.

The role requires strong clinical assessment, communication, and coordination skills to support seamless transitions of care with the interdisciplinary team across settings.

Qualifications

• Graduate of an accredited Vocational Nursing program.
• Current and unrestricted LVN license in the state of practice.
• Current CPR certification.
• Valid driver license and acceptable driving record.
• Minimum one year of nursing experience; hospice/home health intake or admissions preferred.

Responsibilities

• Receive, review, and process hospice referrals from physicians, hospitals, and facilities.
• Verify eligibility and physician orders for admission.
• Maintain referral records in the EMR.
• Communicate with patients, families, and referral sources regarding admission procedures.
• Coordinate admission scheduling with clinical staff to ensure timely transitions.
• Assist in coordinating care with interdisciplinary team and community partners.
