Greenlife Healthcare Staffing is seeking a dedicated Licensed Practical Nurse to join a five-star rated Home Care Facility located in Brooklyn (Strickland), New York. This facility has consistently received top ratings from governmental agencies, reflecting its commitment to providing exceptional patient care.

Schedule of Licensed Practical Nurse:

This is a Full-time / Day Shift (Flexible hours)

Essential Job Duties of Intake Registered Nurse:

• Ensure quality and safe delivery of services by taking referrals and processing appropriate referrals for home health care
• Contact hospital discharge planners, primary nurses, physicians, and case managers as needed to obtain complete referral information
• Develop and enhance relationships with referral sources
• Assess the medical necessity of new referrals and arrange preadmission assessments as necessary to verify eligibility for home care
• Provide complete referral information to community health supervisors and designees
• Accurately take physician or authorized practitioner's orders, verify these in writing, and disseminate orders to appropriate health care personnel
• Complete all required documentation in an accurate, legible, and timely manner in accordance with established policies and procedures
• Involve physicians, patients, and caregivers in developing plans of care whenever possible and appropriate
• Provide information regarding the patient's plan of care to the physician or authorized practitioner and other personnel as appropriate and in a timely fashion
• Complete, document, and submit work as per policy and procedure
• Attend meetings and case conferences as appropriate
• Communicate in a timely fashion all referrals and any changes in patient status to all applicable personnel
• Demonstrate understanding of and compliance with philosophy, objectives, and goals
• Provide accurate information to patients, caregivers, referral sources, and other staff regarding services and associated costs, including knowledge of reimbursement and regulation
• Complete, document, and submit work as per policies and procedures
