Job summary

A leading healthcare provider is seeking a Licensed Practical Nurse (LPN) to join their team in New York. The ideal candidate will have at least one year of nursing experience and be licensed in New York. Responsibilities include conducting health assessments, managing client prescriptions, and coordinating care with healthcare providers. This position offers a Monday to Friday schedule with every other Saturday, starting ASAP.

Qualifications

• At least 1 year of nursing experience.
• NYS Licensed Practical Nurse (LPN).
• BLS certification required.

Responsibilities

• Review potential client medical histories.
• Conduct health assessments with new clients.
• Administer medications and maintain accurate records.

Skills

Knowledge of substance and alcohol dependence/use populations

Bilingual in English and Spanish

About the Opportunity:

• Start Date: ASAP
• Schedule: Monday to Friday, and every other Saturday
• Shift: 6:30am-2:30pm
• EMR: SigmaCare

Responsibilities:

The Licensed Practical Nurse (LPN) will:

• Review referred potential client medical histories submitted by the Intake Department
• Assist with referring clients to appropriate healthcare provider specialists
• Assist with managing the symptoms of withdrawal for clients in the stabilization phase, including referrals for further medical attention
• Conduct health assessments with all new clients
• Assist with managing client prescriptions with various pharmacies
• Administer medications and maintain accurate medication records for review and audit
• Coordinate with primary care physicians regarding management and treatment decisions
• Implement and follow up on the established plan of care and monitor progress
• Assist in client referral and follow-up appointments
• Perform other duties as assigned

Qualifications:

• At least 1 year of nursing experience
• High School Diploma / GED
• Graduate of an LPN program
• NYS Licensed Practical Nurse (LPN)
• BLS certification
• Knowledge of substance and alcohol dependence/use populations

Desired Skills:

• Associate's and/or Bachelor's Degree
• Bilingual in English and Spanish
• Methadone treatment experience
• Knowledge of crisis management techniques for potentially violent behavior
