Job summary

Phoenix Rescue Mission is seeking a dedicated candidate for a 2:00pm-10:00pm shift in its RAP Program, with a $3 shift differential. You will conduct screens, complete intakes, and coordinate care to support clients experiencing hunger, homelessness, addiction, and trauma within a Christian, service-driven environment.

Applicants should have a high school diploma or GED (Bachelor's preferred), plus at least one year in social services, intake, or related fields.

Qualifications

• High school diploma or GED required; bachelor's degree from an accredited university or college in social work, sociology, psychology, counseling, or related social service field preferred.
• Minimum of one year of experience in social services, intake, behavioral health, customer service, or related field required.
• Previous experience in intake, scheduling, and coordination of care is strongly preferred.
• Certification in Peer Support is highly desired.
• Proficient in Microsoft Office software.
• Program database experience preferred.

Responsibilities

• Conduct phone screens to determine prospective clients' eligibility for the RAP Program, gathering detailed information to support appropriate placement.
• Complete intake appointments, including obtaining required documentation, releases, and consent forms while ensuring accurate and timely data entry into the Mission's database.
• Schedule case management appointments and other follow-up services to support a smooth transition into care.
• Conduct property searches and urine analyses in accordance with program policies and procedures.
• Connect clients with appropriate community resources and coordinate external referrals when program eligibility requirements are not met or different levels of care are needed.
• De-escalate crisis situations and communicate effectively with clients, families, and team members to promote positive outcomes.
• Build collaborative relationships with community providers and internal teams to ensure continuity of care and service delivery.
• Demonstrate understanding and comply with all relevant legal, regulatory, and compliance requirements, company policies and procedures, and the Code of Ethical Conduct.
• Follow the Mission's Statement of Faith and lifestyle and adhere to the Mission's employee policy manual and guidelines set forth by the CEO.
• Lead or participate in devotions, prayers, or other organizational activities that promote a credible Christian witness and lifestyle.
• Perform other duties within scope as assigned.
