Lutheran Social Services of Illinois is seeking a full-time Intake Coordinator to support admissions into Residential Rehab. The role involves completing assessments, treatment plans, and related documentation, with a focus on timely processing and collaboration with clinical staff.

This position requires prior experience in substance use services, a valid driver’s license, and proficiency with EHR systems. The organization offers comprehensive benefits and a supportive work environment.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s or master’s degree in a social service field preferred.
• CADC or LCSW/LCPC preferred.
• Background check clearance required.
• Valid driver’s license and reliable transportation required.
• 3–5 years of experience in a substance use field preferred.

Responsibilities

• Complete LOC and send to supervisor within one day of admission.
• Obtain final signature on LOC within 72 hours of admission.
• Close Assessment Program and open client into Residential Program in the EHR after LOC.
• Complete Treatment Planning Assessment within 24 hours and obtain signatures within 72 hours.
• Prepare initial treatment plan within 48 hours and obtain reviews and signatures within 72 hours.
• Document treatment plan reviews within seven calendar days of signature from last credential.
• Assess client needs, identify appropriate treatment options, and coordinate linkage to care.
• Prepare required paperwork for screenings, assessments, and discharge criteria accurately and timely.
• Maintain professional conduct and respectful interactions with clients and colleagues.
