RiseWell Community Services is seeking a compassionate Full-Time Intake Coordinator to join our PROS Program in Copiague, NY. You will screen and conduct intakes, assess needs, and develop recovery plans for individuals entering the PROS program.

Qualifications

• Licensed Master Social Worker (LMSW) credential required.
• Experience working with individuals in recovery from mental illness.
• Valid and clean NYS driver's license.
• Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

Responsibilities

• Screen and conduct intakes for individuals seeking admission to the PROS Program.
• Conduct assessments to determine needs and preferences and develop recommendations for Individual Recovery Plan.
• Provide new member orientation and coordinate with referral sources and collaterals.
• Maintain appropriate records according to program regulations.
