RiseWell Community Services is seeking a compassionate Full-Time Intake Coordinator to join our PROS Program - Personalized Recovery Oriented Services in Patchogue, NY.

In this role, you will be responsible for screening and conducting intakes for individuals seeking admission to the PROS Program. Conduct assessments to determine individuals’ needs and preferences and develop recommendations for the person’s Individual Recovery Plan. Conduct new member orientation and coordinate with all referral sources and collaterals. Maintain appropriate records according to program regulations.

Qualifications

• Licensed Master Social Worker
• Experience working with individuals in recovery from Mental Illness
• Valid and clean NYS Driver’s License
• Fingerprinting, criminal record check, approval from NYS Office of Mental Health
