Job summary

Calvary Healing Center, part of Universal Health Services, is seeking an Intake Coordinator to facilitate admissions for prospective patients. You will manage referrals, coordinate assessments and admissions, and complete required documentation, partnering with patients, families, providers, and community partners to ensure timely placement.

Under the Intake Director, you will support a full continuum of care in addiction treatment across Phoenix, with on-site detox, inpatient, PHP and IOP

Qualifications

• Must be at least 21 years of age, without a professional licensure.
• High school diploma or GED required.
• Bachelor’s degree in Counseling, Psychology, or Human Services preferred, or 1 year experience in a related field.
• Valid Fingerprint Clearance Card.
• Completion of BLS and First Aid Certification within 30 days of employment.
• Completion of Verbal De-Escalation training within 30 days of employment.

Responsibilities

• Facilitate the admission process for prospective patients.
• Manage incoming referrals, coordinate assessments and admissions, and complete required admission documentation.
• Collaborate with patients, families, providers, and community partners to support timely and appropriate placement.
• Work under the direction of the Intake Director.
